Аннотация
Более половины всех злокачественных опухолей панкреатодуоденальной области представляют злокачественные новообразования поджелудочной железы. Актуальность проблемы хирургического лечения опухолей данной локализации связана не только с распространенностью и жизнеугрожающим характером заболевания, но и с неудовлетворенностью результатами данного лечения.
Представлен анализ современных данных об осложнениях операций по поводу злокачественных новообразований поджелудочной железы.
Рассмотрена современная тактика при замедленном опорожнении желудка, формировании внутрибрюшинного абсцесса, развитии пострезекционных кровотечений (в зависимости от сроков возникновения кровотечения после операции на основе специализированной классификации, предложенной в 2007 г. Международной исследовательской группой по хирургии поджелудочной железы (ISGPS), а также при несостоятельности гепатикоеюноанастомоза. Особое внимание уделено патогенезу и особенностям лечения панкреатической фистулы.
Несмотря на ряд противоречий во взглядах, авторы сходятся во мнении о большой роли периоперационного периода и необходимости индивидуализации тактики ведения пациентов с помощью шкал оценки факторов риска формирования тех или иных послеоперационных осложнений.
